Version bump and changelog for 0.4.0. After merging: tools/release.sh --tag 0.4.0

• Update dependencies (2026-10-19) (#140)
• Say why when the relay refuses the connection (#142)
• Desktop smoke: press space again if no channel was entered (#144)
• Reconnect: time out a stuck login, close failed attempts, log why (#145)
• cargo-deny: retry when fetching the advisory database fails (#143)
• DOCKER.md: reaching the home container over Tailscale (#146)
